$112.50 per 45-minute session
$150 per 55-60-minute session
$187.50 per per initial session (75 minutes)

Insurance

I am an In-Network provider (Preferred Provider) for the following health insurance plans : 

• Blue Cross/Blue Shield
• Kaiser Permanente (PPO)
• Lifewise
• Premera Blue Cross

I am an Out-of-Network provider and will bill on your behalf the following health insurance plans:

• Aetna
• Cigna
• Optum
• Regence BlueShield
• Uniform Medical Plan
• United Behavioral Health
• United Healthcare

For other health insurance plans, please discuss billing arrangements with me. For all insurance plans for whom I am out-of-network, you will will pay me for the session and will be reimbursed by your insurance plan according to your benefits and their policies. 

Reduced Fee 

Reduced fee services are available on a limited basis 

Payment
Cash, check and all major credit cards accepted for payment.

Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified me at least 48 hours in advance, you will be required to pay the full cost of the session.
